1.     What is an FQHC?

 

An FQHC is a Federally Qualified Health Center. As an FQHC, our goal is to provide quality, affordale, and accesible health care to the community surrounding Sayre High School. We offer comprehensive Services to our patients.

 

2.    Where are you located?

 

We are located at:

We are easily accessible:

  • By car: Entrance to the parking lot is at 58th and Locust Streets

  • By bus: 21, 31, 42, or G routes to 59th Street

  • By train: 60th Street Station on the Market-Frankford Elevated Line (The El or Blue Line)

 

3.    What are your hours of operation?

 

We are open Monday, Wednesday, and Friday 8:00 AM to 5:00 PM. We are open Tuesday and Thursday 8:00 AM to 8:00 PM.

5.    Who can be seen at Sayre Health Center?

 

We provide comprehensive health care for people of all ages based on a Family Medicine model of primary care. We provide integrated behavioral health services as part of our primary care program. 

 

6.    Do you take my insurance?

 

We accept a number of commercial insurances, medicaid, and medicare. For more information, please click here or call (215) 474-4444 and we will be happy to assist you!

 

7.    What does underinsured mean?

 

Those who are underinsured have some form of health coverage, but there are limitations to what services are available to the patient. Out of pocket costs are limited to the cost of a visit with no insurance. Patients only have the out of pocket cost that our sliding scale indicates. This applies to individuals with underinsurance or high deductible health plans. 

 

8.    Are you a free clinic?

 

No we are not a free clinic. However, we do offer services on a sliding scale fee to patients who are uninsured or underinsured. We do have some services that are free to all such as Covid-19 testing and vaccines and flu shots.

 

9.    What is the cost of my visit?

 

The cost of your visit will depend on what insurance you have. If you are uninsured or underinsured, we offer a sliding scale fee based on household income. Proof of income will be required to determine payment. If you have any additional questions, please call us at (215) 474-4444

 

10.    Do you take walk-ins?

 

Yes, we do take walk-ins. Patients can be seen in person or via a virtual visit. Our online clinic can be found at sayrehealth.doxy.me/checkin

11.    Do you have same day scheduling?

​

Same day scheduling is available! This is dependent upon the reason for the visit and physician availability. For more information, please call (215) 474-4444

 

12.     How do I make an appointment?

​

Making an appointment is easy. Just call us at (215) 474-4444 and one of our scheduling specialists will be happy to assist you.

13.     How do I cancel my appointment?

​

Please call us (215) 474-4444 if you need to cancel or make changes to your appointment.

​

14.    Do you have specialists on site?

​

No, we do not have specialists here. However, if after an appointment it is decided by one of our physicians that you need to see a specialist, you will be referred to one. Some specialist provide service to Sayre patients via telehealth. 

 

15.    How can I communicate with my physician or other members of the clinical team during business hours?

​

You can call (215) 474-4444 and leave a message or fax a request to (215) 474-6021. The physician or someone from the clinical team will respond to your request as soon as they can.

​

16.    What if I need to talk to my provider when the office is closed?

​

For emergencies, you can still call (215) 474-4444 after regular office hours and leave a message for the on-call physician. Please note, the on-call physician may or may not be your physician. The on-call physician will receive the message and respond to the request.

17.    What if I need a medication refill?

​

Your pharmacy can request refills on your behalf. You can also call in your request. Since medication refill requests can take 2-4 business days to be filled, please be sure to call before your medication has run out. Prescriptions for controlled substances must be filled in person, during a visit.

18.    What if I need a form completed?

​

Forms can be dropped off in person or faxed to (215) 474-6021. Forms will be completed within 5-14 business days from date of receipt. If you do not have a recent visit, you will be asked to make one.

​

19.    How do I obtain a copy of my medical records?

​

You can call or come in to our office to request a copy of you record. Medical records will be ready for pickup within 10-14 business days of the initial request. Please remember to bring a photo ID when you pickup your medical records

 

19.    How can I volunteer?

 

Sayre Health Center has events, community health programming, and conducts wellness assessments in our service area throughout the year. Volunteers are always welcomed and appreciative. If you are interested in revitalizing community health and want to learn more about how you can volunteer, please email our Outreach department at outreach@sayrehealth.org

 

20.    How can I donate?
